Izumi’s dream was to have children and always to be happily married to Itachi. When an order was issued to Itachi to go through with a massacre, little did he know that one of the clans to be massacred was Izumi’s clan.
Due to his love for Izumi, Itachi had doubts about going through with the order to kill Izumi’s clan members.
To ensure the completion of his mission; he chose to kill Izumi first in order to cast off any doubt. Izumi died by itachi’s hand and he thanked her for her love as she lay dying in his arms.
On the night of the Uchiha Massacre, Izumi was lured away from home by Itachi and he later trapped her with one of the most powerful Jenjutsu called Tsukuyomi that put her in The Illusion where she experienced her entire alternate life in a single moment.
In the anime Naruto, Izumi was murdered by Obito. However in the official series, it was Itachi who killed her.

The painful death of Izumi took place in the “Moonlit Night” installment, listed as episode 455 of the anime series Naruto.
Izumi was the daughter to Hazuki Uchiha. Her father died while protecting her during the attack of the Nine-Tailed Demon Foxes.
She was half Uchiha because her father was not originally from the Uchiha Clan. As a result, Izumi was not considered an official member of the clan.
Since childhood, Izumi had always been infatuated with Itachi, which later blossomed into true love. Itachi, with time, also came to love Izumi.
Her true love for Itachi enabled her to accept Itachi’s decision to take her life and to go through with the Uchiha Massacre.
In The Illusion, Izumi becomes a ninja before retiring from her Shinobi life to get married to Itachi. They have children and Izumi dies of old age alongside Itachi at the age of eighty years.
Izumi was able to experience all those years through The Illusion while only a few seconds had actually lapsed in the real world.
Immediately Izumi regained her consciousness from her suspension in The Illusion induced by Itachi, she found herself in Itachi’s hands on the brink of death. During her last moments, she thanked Itachi, as her life slowly drifted away from her frail body.
Even though the series does not take place in actual parts within Japan, Naruto features real life ninjas who once lived and worked in old Japan.
